Form(ula) for Documentation of Social Security Benefits

SOMERVILLE – Freeholder Robert Zaborowski, human services liaison, and the Somerset County Department of Human Services invite social workers and case managers to a free workshop about Social Security disability benefits for adults and effective methods of completing required documentation. The program will be held from 8:30 a.m. to 12:30 p.m. on Thursday, June 12.

Sarah Dubinsky, staff attorney at the National Organization of Social Security Claimants’ Representatives, will provide an introduction to Social Security disability benefits and demonstrate how to advocate in writing for patient benefits. Participants will learn how to document medical records, assist clients with forms and how to complete forms on behalf of patients.

The program will be held in a first floor conference room at the Somerset County Human Services Building, located at 27 Warren Street. Parking is available in the adjacent parking deck, third level or higher.

This course is approved for 3.5 NASW-NJ continuing education credits. The approval period is through August 2014.
